הצהרת טוהר בחינות

התחייבות הנבחן: הנני מתחייב כי קראתי והבנתי את כלל ההנחיות המפורטות בנספח זה (נספח 2 – הנחיות לנבחנים בבחינה מקוונת), והנני מתחייב לפעול בהתאם הנחיות אלה. ידוע לי כי באם לא אפעל לפי ההנחיות המפורטות, הנני צפוי לטיפול משמעתי בהתאם לנהלי הטכניון כפי שמופיעים בתקנון המשמעת. כמו כן, הנני נותן בזה את הסכמתי המלאה והבלתי חוזרת לצילום ותיעוד של מהלך הבחינה, בין אם באמצעות תוכנת ה- Zoom ובין באמצעות אפליקציה אחרת, כפי שתעודכן מעת לעת בנהלי הבחינה. ידוע לי כי הצילומים משמשים אך ורק לצרכי מעקב, ניטור וניהול תקין של הבחינה וכי הללו ימחקו בהתאם לנוהל שמירת מחברות המופיע בנהלי הטכניון. יובהר בזאת כי חתימתי זו מהווה אישור לכלל המבחנים המקוונים להם אדרש במהלך תקופת לימשודיי בטכניון.

I hereby guarantee that I have read and understood all of the guidelines set forth in this Appendix (Appendix 2 - Online Exams Guidelines for Students)and I commit to follow these guidelines. I am aware that if I do not follow them, I may face disciplinary action in accordance with the Technion's Disciplinary Rules. Also, I give my full and irrevocable consent to photography and documentation of the course of the exam, whether through the ZOOM software or through another app, as updated from time to time in exam procedures. I know that photographs are used solely for tracking, monitoring purposes and proper management of the exam and that these will be deleted in accordance with the retention procedure of companies listed in the Technion's procedures. It is hereby clarified that this signature constitutes approval for all the online tests that I shall require during my period of study at the Technion.

לצורך קיום הבחינה מרחוק באמצעות היוועדות חזותית, הנך מסכים ומאשר כדלקמן:

במהלך הבחינה עליך לשהות בחדר ריק מכל אדם אחר ונקי מכל מסמך, צילום או כל פריט אחר אשר יש בצילומו כדי לפגוע בפרטיותך או בפרטיות של צד שלישי כלשהו. ידוע לך שמהלך הבחינה תתועד בהיוועדות חזותית ותוקלט על ידי המשגיח וזאת לצורך שמירה על טוהר הבחינה. עם קבלת תוצאות הבחינה ושלילת חשש לפגיעה בטוהר הבחינות, תמחק ההקלטה ללא צורך במתן הודעה מוקדמת. במקרה של חשד לפגיעה בטוהר הבחינות, תישמר ההקלטה עד לסיום בירור הטענות. בכל שאלה או טענה הנוגעת לשמירה או פגיעה בפרטיות בקשר עם האמור לעיל, ניתן לפנות לממונה על ההגנה על הפרטיות בכתובת הדוא"ל dpo@technion.ac.il.

For the purpose of taking the exam remotely through video conferencing, you agree to and confirm the following:

During the exam, you must remain in a room that is devoid of any other people and that does not contain any documents, photographs, or any other item that may violate your privacy or the privacy of any third parties. The entire duration of the exam will be recorded by the proctor via video for  the purpose of maintaining the integrity of the exam. Upon receipt of the exam results, and so long as there is no concern of a breach of exam integrity, the recording will be erased, without any advanced notice. In the case of any suspected violation to the integrity of the exam, the recording will be retained until the suspected violations have been sorted. When submitting the exam, you will be required to attach a copy of your ID for identification purposes and to connect between the student and the exam that he has submitted. Upon receipt of the exam results, and so long as there is no concern of a breach of exam integrity, the copy of the ID will be deleted, without any advanced notice. In the case of any suspected violation to the integrity of the exam, the documents will be retained until the suspected violations have been sorted. For any questions or complaints regarding the protection, or breach, of privacy in regard to the aforementioned points, you may contact the Privacy Protection Office at dpo@technion.ac.il.

ויתור על בחינה

אני מוותר על בחינה זו מרצוני האישי ומודע להשלכות של מעשה זה כפי שכתוב בהנחיות לבחינות המקוונות סעיף 27.

I waive this exam of my own free will and I am aware of the consequences of this act as stated in the Guidelines for Online Exams Section 27.
